It gives me great pleasure to welcome you to the 6th scientific symposium of SFG of SSOG.
The three main tasks of the meeting are:
• First, is to provide us with an opportunity to share experiences and update our knowledge for the benefit of our practices and clients.
• Second, is to keep the wheel of CME and active research running for the progress in reproduction health and medical science.
• Overall, task is “social” as by meeting old friends or making new ones, the bond grows stronger among us and between generations.

Is a Consultant and Head Section of Reproductive Medicine, Department of Obstetrics and Gynecology, King Faisal Specialist Hospital & Research Center (KFSHRC), Riyadh. He obtained MBBS degree from King Faisal University, Dammam. He has been trained in the British Isles and worked there for more than 14 years. Certificate of Full Registration as a Medical Practitioner was granted by General Medical Council (GMC). Initially he got the Licentiate of Midwifery (LM), The Rotunda Hospital, Dublin, Ireland. He obtained the membership (MRCOG), then the Fellowship (FRCOG) of the College of Obstetricians and Gynecologists, London, UK. He gets Certificate for Subspecialty of Reproductive Medicine, Nottingham University Research and Treatment Unit in Reproduction, U.K. In addition to the clinical works, a Master of Medical Science in Assisted Reproduction Technology (MMSci-ART) obtained from Nottingham University, Nottingham, U.K.
Postgraduate teaching is one of his interests and abilities. He has been involved in theoretical and bedside teaching for the last 30 years. He is an Examiner and an Author of Part I and II Examination for Saudi Specialty Certificate of Obstetrics and Gynecology (SSCOG) since 1999 to date.
He was the Director of Residency Training Program, then the Director Fellowship Program, Reproductive Medicine, Department of Obstetrics and Gynecology KFSHRC. He was a member in the Saudi Council for Health Specialties (SCHS), Scientific Board of Obstetrics & Gynecology.
Currently he is a Member of the Infertility Practice Regulations Committee, Ministry of Health and Chairman of Infertility Practice Evaluations subcommittee, Ministry of Health. His interest is in ethical and religious issues related to the practice in Reproductive Medicine especially in Saudi Arabia.

Is a senior scientist in the field of In Vitro Fertilization and Pre-Implantation Genetic Diagnosis, She is the managing director of Al-Jawhara Hospital for Fertility and Genetic Diagnosis and an assistant professor in the Department of Molecular Medicine, College of Medicine and Medical Sciences, Arabian Gulf University.
Over the last 22 years, she has attended more than 30 hands on workshops in Andrology, IVF, and ICSI, in-vitro maturation of oocytes and slow and rapid cryopreservation techniques of sperms, Embryos, testicular tissues and PGD.
Dr. Dashti is also involved in ISO 9001/ CAP for quality assurance in IVF and she is an active participant in presenting and publishing documents in relation to the bioethical issues in research and application of ART/ molecular biotechnologies for UNICEF and UNESCO.
She was an active member and chairperson in many societies of reproductive medicine.

Dr. Alfadhel is currently the Head of Genetics Division, Department of Pediatrics at King Abdulaziz Medical City, Riyadh, Saudi Arabia and an associate professor at King Saud bin Abdulaziz University for Health Sciences (KSAU-HS). Dr. Alfadhel received his bachelor degree in general medicine and surgery from King Saud University in Riyadh. He obtained his pediatric residency from King Abdulaziz Medical City in Riyadh and received the Pediatric Saudi Board certificate followed by Pediatric Arab Board certificate and the Canadian Board in Biochemical Genetics. In addition, he carries a Neurogenetics fellowship certificate and Master of Health Science (MHSc) in epidemiology. He is also a Chair at the Newborn Screening Program of King Abdulaziz Medical City, member of advisory board of national Newborn Screening Program and member of Middle East Hyperammonemia and UCDs Scientific Group (MHUSG).
Dr. Alfadhel’s foresight and passion for innovation, earned him an award for inventing a new application in the Apple store for the treatment of inborn errors of metabolism. He has published several research papers in the field of Genetics disorders, and participated in several conferences and symposia. Dr. Alfadhel’s current research interest is focused on studying genetic disorders and understanding the underlying molecular pathways that play a major role in some brain disorders and other genetic diseases. He was successful in contribution in discovery of 35 novel genes specifically for disorders involving the brain.

Dr. Javed has 37 years experience in Embryology career. His educational qualifications are; DVM, MSc (Hons), PhD and 2 Post-Doc Fellowships. His education is from world renowned and accredited universities. His clinical research career in Reproductive Physiology started in 1981. Until 1996 (16 years) he remained exclusively in Embryology research. In 1997, he joined clinical human Assisted Reproductive Technology Laboratory in Canada and since then he has been in this profession (21 years).
Dr. Murid Javed became certified Director of human Assisted Reproductive Technology Laboratory in 2013. This certificate was awarded by Canadian Fertility and Andrology Society which is professional certifying organization in Canada. Dr. Javed also holds highest Embryology certification (EMB) from American College of Embryology. He is also member of the Practice Committee and Chair of the Certification Committee of this College. He has been member of several professional societies including ESHRE, ASRM, CFAS, Alpha Scientists in Reproductive Biology. He is Life Member of Pakistan Veterinary Medical Association.
Dr. Javed joined Thuriah Medical Center in Oct 2015 as IVF laboratory Director. Earlier, he has worked in USA, Canada, Japan, China and Pakistan.
Dr. Javed has extensive research and clinical experience in all aspects of Assisted Reproductive Technology. He has published extensively and has written a book chapter. He is on editorial board of 5 scientific journals. He has served on committees MSc and PhD students. He has presented his work orally and in poster format in many international scientific meetings. He likes to share knowledge, train new staff and implements all measures for best care of sperm, eggs and embryos in the laboratory.

A Family with Female-limited infertility caused by oocyte maturation failure

The syndrome of oocyte maturation failure is an extremely rare cause of primary female infertility with only a few cases reported to date. This maturation arrest can represent failure to complete any of the various stages of meiosis I or II. Thus, it remains unknown what causes maturation arrest in human female individuals with infertility.
In this case study, we report a 23 years old lady, presented with 7 years primary infertility to our IVF center at KFSHRC. She had undergone seven failed IVF cycles, each with adequate number of retrieved eggs (5-23 oocytes) but all arrested in meiosis I as indicated by the presence of germinal vesicle. Oocytes were cultured in the in vitro maturation media without any progress on meiotic maturation for 48 hrs. Hormone levels were measured on the 3rd day of the cycle and her FSH level is 6.6 IU/L and there was no indication of polycystic ovary disorder on hormonal or imaging studies.
She gave history of similar infertility concerns in one of her younger sisters. This sister also has history of 6 years of primary infertility. She had undergone one IVF cycle and 4 retrieved eggs were arrested in meiosis. Like her sister, she lacked features of polycystic ovary disorder, and her FSH level was 5.6 IU/L on the 3rd day of the cycle.
Although consanguinity was denied, both parents belong to the same tribe Positional mapping and whole-exome sequencing revealed one rare homozygous variant involved gene PATL2
In conclusion, we suggest that meiosis I arrest may be caused by PATL2 mutation leading to female-limited infertility in humans. This rare etiology of infertility expands our knowledge of factors required for normal human oogenesis. It is hoped that this and future discoveries of the molecular underpinning of human infertility will inform and advance new therapeutic strategies.

Bioethics of genetic testing in relation to the outcome in fertility programs

Pre-implantation Genetic Screening/Diagnosis (PGS/PGD) is defined as a group of genetic analysis applied to embryos produced through IVF programs for families carrying or showing some genetic disorders. Also, it is an efficient tool to enhance the chances of healthy pregnancy in women suffering from infertility due to recurrent abortions. PGS/PGD tests are mainly performed on embryos and may include their parents depending on the type of abnormality sought. Some tests detect chromosomal aneuploidies while others are concerned with DNA analysis to find defective gene that may produce a disease.
The main issues of PGS/PGD in any society can be summarized as following:
  • They are extremely complex, laborious and costly. Therefore, labs performing them should be subject to strict regulations to ensure accuracy and reliability of test results.
  • Clinics offer genetic testing of embryos should consider counselling as a mandatory part of PGS/PGD practice.
  • Children produced through PGS/PGD must be registered and followed up to ensure their health and welfare.
  • The potential impact of PGS/PGD on female pregnancies and termination in pro-male societies.
  • The potential impact of PGS/PGD on discrimination of the disabled.
  • Effect of X-linked diseases on marital status of women

Ethical problems in PGS/PGD arise when any of the above points is ignored.
In view of the above enumerated issues, this study will present the outcome of a sample of 100 patients undergone PGS/PGD testing, in relation to their expectations, high cost, psychological stress, success and may be to total disappointment.

As medicine has evolved over the last century, medical genetics has grown from nonexistence to one of the most visible aspects of how we understand and treat disease. Genetics has emerged as a primary focus of clinical needs with translational applications within reproductive medicine. The aim of this lecture to outline the applications of genetics currently available, and how these technologies can provide a positive impact on patient care. Additionally, will give an idea about preimplantation Genetic screening (PGS), preimplantation Genetic diagnosis (PGD) the differences between them and what is their challenges to Obstetricians and professionals working in that field.

Disorders of sperm quality range from a low count or motility to a complete absence of sperm production.

Preimplantation genetic diagnosis (PGD) is an important method for the identification chromosomal abnormalities and genes responsible for genetic defects in embryos that are created through in vitro fertilization before pregnancy. As the list of conditions and indications for PGD testing is continuing to extend enormously, novel in vitro fertilization techniques and newly established genetic analysis techniques have been implemented in clinical settings in the recent years. Blastocyst-stage biopsy, vitrification techniques, time-lapse imaging, whole-genome amplification, array-based diagnostic techniques, and next-generation sequencing techniques are promising techniques for the accurate diagnosis of diverse genetic conditions and also for the selection of the best embryo that has the highest implantation capacity. The timing and technique used for biopsy, the amplification techniques, the genetic diagnosis techniques, and appropriate genetic counselling play important roles in establishing a successful PGD

Ovarian Stimulation for IVF/ ICSI.

Ovarian stimulation is crucial to the success of IVF/ICSI treatment. Although it is generally safe and effective the safety challenge can be frequently encountered in patients with high ovarian reserve and the effectiveness can be also challenged in patents with reduced ovarian reserve or in some cases reduced sensitivity to exogenous gonadotropin stimulation.
Over the past decades several stimulation protocols have been used and accumulated experience has suggested that the two main protocols that are currently being used worldwide are the agonist long protocol and the antagonist short protocol. Whilst the two protocols can be a suitable alternative to each other in the majority of clinical scenarios there are circumstances where the use of one is preferred to the other. Individualization of stimulation is currently being promoted in order to cater for the variable requirements of different categories of patients. This may involve using particular gonadotropin preparation in either individually or in combination with other preparations to obtain the best outcome.
In this lecture the focus will be on the various stimulation protocols and their most appropriate use in order to optimize outcomes for different clinical categories of patients.

Long acting Corifollitropin alfa (Elonva) stimulation in normal and hyper responders. How can we do it safely?

Between May 1ST 2012 and May 1st 2016the total of 615 case of IVF were retrospectively reviewed and divided into 2 groups: Group A (Elonva): 173 cases and out of this group 44 were PCOS cases. Group B (non-Elonva): 442 cases were treated with daily injections of gonadotrophins with ( a short fixed antagonist protocol ), 39 were Cases of PCOS.
Corifollitropin alfa, self- administered subcutaneous injection in abdominal wall. Risk of OHSS can be avoided by using antagonist protocol with GnRH-a Ovulation triggering instead of HCG. Compare even with same protocol. No difference in pregnancy outcome. corifollitropin alfa could decrease the injection burden of patients undergoing IVF/ICSI treatment. Can give the patients a chance for cryopreservation of the extra-embryos formed.

Conventional ovarian stimulation protocols for IVF, using high doses of gonadotropins are designed to achieve maximum oocyte yields. While mild stimulation protocols used lower dose of gonadotrophins to achieve a less numbers of oocyte
Reports are conflicting on the effects of mild versus conventional stimulation on embryo quality.
This presentation will compare the pros and cons of each stimulation with regard to embryo quality.

Despite the advances in Assisted Reproduction Techniques (ART) success rates of IVF / ICSI remain generally below expectations. It is therefore so common that patients need to undergo repeated IVF/ICSI attempts in order to try to achieve their goal of having a baby. Resilience is key to enable patients to continue with their journey, with trying more than one attempt of treatment. Financial resilience is an important aspect of this exercise. Discussing the tools of reducing the costs of IVF treatment without undermining quality would be the subject of this lecture.

Unexplained infertility refers to failure to achieve pregnancy after 12 months of unprotected intercourse where the standard infertility investigations failed to identify the cause of infertility.
It affects about 15-30% of subfertile couples and it is probably due to subtle defect in the couple life style, oocyte, sperm or endometrium.
The management of couples with unexplained should take into consideration the cost, safety and risks of various treatments and should be individualized for each couple. The possible options are change in life style, expectant management, ovulation induction, IUI and IVF.
